SKILLED NURSE VISIT Private Duty Home Based Nurse

The responsibilities of the SKILLED NURSE for a home visit may include:

• Obtaining vital signs and notifying the doctor when outside of normal range
• Setting up pill box and ensuring patient is taking medications as ordered
• Helping to set up doctor appointments as needed
• Providing education in health, nutrition, diabetes and chronic disease management
• Personal companionship and patient advocacy
• Simple wound care and dressing changes
• Skin assessment to ensure integrity of skin
• Informing the caregiver or family of any immediate health concerns
• Assistance in changing ostomy bag/appliance, provide teaching if needed

There is no long term contract needed. These are set up as episodic visits, as needed. Visits will be scheduled for 1-2 hour blocks. Home visits need to be scheduled at least 1-2 weeks in advance and paid in full prior to visit. The visit is non refundable in the event that the patient is not home, unavailable, refuses visit, sick, no one answers the door. PETS in particular DOGS need to be put away during the visit to ensure safety of the nurse.

Requirements: Caregiver/DPOA will need to sign a consent giving permission for treatment. A HIPPA consent is also necessary.

These type of visits are private duty and not paid by insurance. Insurance will not be billed.
